WorkCover WA Announces Updated Forms Following Stakeholder Consultation


WorkCover WA has finalised changes to a number of approved forms as part of its ongoing implementation of the Workers Compensation and Injury Management Act 2023, which came into operation on 1 July 2024.

The updated forms follow a comprehensive review and consultation process aimed at ensuring consistency with the new legislative framework. 

NIBA engaged with WorkCover WA throughout the review, providing feedback on the practical application of the proposed changes and their impact on the broking profession and clients and raising a number of issues for further discussion with the regulator. 

The updated forms, listed below, will come into operation on 1 July 2025: 

  • SF1 – Settlement Agreement 
  • SF3 – Permanent Impairment Notice 
  • CN1 – Intention to Reduce or Discontinue Income Compensation – Consent 
  • CN2 – Intention to Reduce or Discontinue Compensation – Return to Work 
  • CN6 – Custody or Imprisonment Notice 
  • APIA3 – Permanent Impairment Assessment Requirement to Attend 
  • APIA4 – Permanent Impairment Assessment – Provision of Information 
  • APIA5 – Permanent Impairment Assessment Consent to Provision of Information 
  • CF2 – NIHL Compensation Claim Form 
  • NIHL1 – NIHL Audiological Test Report 
  • NIHL3 – Worker Noise Exposure and Employment History 

The new forms are now available on the WorkCover WA website. While current versions of these forms remain in use until 30 June 2025, brokers should ensure that the new versions are adopted from 1 July 2025.
